Project narrative

The ageing European population presents profound challenges, with rising chronic illness, mental health conditions, and disabilities leading to high societal costs. Dementia prevalence escalates notably in older age groups, impacting even those with early-onset dementia. Early detection proves crucial for effective planning and interventions, necessitating integrated healthcare knowledge and community-based strategies. Fragmented data and a lack of personalized approaches impede identifying risk factors and tailored treatments, calling for unified data sources (lifestyle, genomic, patient-reported) and tech like AI and IoT for precise risk prediction and interventions. COMFORTAGE aims to craft holistic healthcare models for personalized dementia and frailty prevention, amalgamating clinical expertise, behavioral science, and cutting-edge technology (explainable AI, virtual assistive tools). COMFORTAGE will establish standardized Holistic Health Records (HHRs) and it will validate and evaluate the project’s framework in thirteen pilot studies across eight European countries.  

We will lead the co-creation of the people-centred approaches and will contribute to the assessment of the societal impact of the project’s solutions. Additionally, we will provide guidelines and methodologies for the organization, implementation, and reporting of co-creation and co-design sessions and activities. Finally, WR will also be involved in the design and implementation of advanced social learning mechanisms for enhancing awareness and citizen engagement.
